TERMS OF ADOPTION
1. Care of Our Rescues: As rescue animals, our pets have already been through enough heartache in their lives. Pets
adopted from Dog Pack Rescue will not be used for animal research or experiments, breeding, attack training, dog fighting,
as food for human consumption or that of other animals or for animal sacrifices; they will not be tied up or chained, confined
exclusively outdoors, abused, neglected, given to another owner, or abandoned to the streets. Adopter understands Safe Haven
Animal Rescue Effort, Inc. will never place a known biter in a home with children, however, it should be understood that any
pet may bite if provoked. Adopter also understands that no one can predict how a pet will react to an individual child. Therefore,
adopter will use due caution
when allowing any child to be near this pet, and will never allow any child to threaten, squeeze, poke, step on, or otherwise
pose a perceived threat to this pet.
2. Adoption Fee Refund: The adopted pet MUST be returned to Safe Haven Animal Rescue Effort Inc if the adopter is
not satisfied with any aspect of the adoption. We will be allowed you to exchange the dog or receive a refund minus a $25
processing fee, if the dog is returned to US on or before the 14th day following the adoption. YOU MUST BRING THE DOG TO US,
we will not pick up the dog. NO refund will be given if returned after 14 days from signature date on this agreement.
3. Transfer of Ownership: The adopted pet may not be transferred to any other person for any reason. The adopter
agrees that he/she is not acting as, or in part of any agent in the purchase of this pet, and that the adopter will not sell
this dog to any other person, agent, or pet store. If the adopter is forced to relinquish custody of the pet at any time,
said pet will be relinquished only to Safe Haven Animal Rescue Effort Inc.
4. Reimbursement: There will be no reimbursement for any expenses incurred by the adopter for the adopted pet,
even if it is returned to Safe Haven Animal Rescue Effort Inc.
5. Health and Welfare Program: The adopted pet will be cared for humanely, including but not limited to adequate
food, shelter, water, year round heartworm preventative for the life of the pet, treatment for prevention of internal and
external parasites, fleas, ticks, and routine and emergency veterinary care. Booster vaccinations shall be administered at
least once a year with a routine examination. The pet will be licensed in accordance with the laws in the jurisdiction in
which s/he resides. The pet is to be exercised in a fenced yard or on a leash and NOT TO BE FREE TO ROAM THE STREETS.
6. Identification: Adopter agrees to either micro chip the dog or make sure a tag for identification purposes
be placed on the collar with phone number and address of the dog.
7. Loss: If the adopted pet is lost or stolen, the adopter will notify Safe Haven Animal Rescue Effort Inc immediately
so we can use our resources to help locate the pet as soon as possible.
8. Reservation of Rights: Safe Haven Animal Rescue Effort Inc. reserves the right to examine, make inquiry, and to
request updates about said pet at any time. If the terms and conditions of the Adoption Agreement are not upheld, Safe Haven
Animal Rescue Effort Inc. reserves the right to terminate the Agreement and to reclaim the pet.
9. Notifications: Adopter agrees to inform Safe Haven Animal Rescue Effort Inc. of any address change. The adopter
will also inform Safe Haven Animal Rescue Effort Inc of the pet’s death as well as the cause of death.
10. Permissions: Adopter grants permission for Safe Haven Animal Rescue Effort Inc to discuss this pet with adopter’s
veterinarian. Adopter also gives his/her current or past Veterinarian permission to release any and all medical information
to Safe Haven Animal Rescue Effort Inc about this pet, or any animal that I have owned, do own, or ever will own.
11. Indemnities: Safe Haven Animal Rescue Effort Inc. makes no representations or warranties concerning the health,
temperament and/or previous ownership of the pet. Adopter understands that Safe Haven Animal Rescue Effort Inc shall not be
liable for any medical care or treatment of the pet. Adopter agrees to indemnify and hold harmless Safe Haven Animal Rescue
Effort Inc. and/or their representatives, for and against any and all manner of actions and causes of actions, suits, debts,
dues, accounts, bonds, covenants, agreements, judgments, claims, and demands whatsoever arising out of or relating to the
adoption, placement, and/or possession of the pet.
12. Revisions: Changes, additions, or any other alterations of this contract MUST BE IN WRITING and signed and agreed
upon by ALL PARTIES INVOLVED.
13. Spay and neuter.
If pet is not spayed or neutered at time of adoption I agree to have said procedure taken care of within 30 days of
adoption or when pet has reached the appropriate age to be spayed or neutered. SHARE reserves the right to reclaim any
animal that has not been spayed or neutered within the above specified time frame.

14. Failure to Honor Contract: Failure to honor any part of this contract will result in a penalty of not less than
$1,000.00 per occurrence payable to Safe Haven Animal Rescue Effort Inc.. ANY AND ALL LEGAL FEES AND COURT COSTS NECESSARY
TO ENFORCE THIS CONTRACT WILL BE THE OBLIGATION OF THE ADOPTER INCLUDING ALL THOSE INCURRED BY Safe Haven Animal Rescue Effort
Inc.
